Character Brush

Hi all.
I have a 21 month old son and he HATES brushing his teeth.
I have tried:
singing songs to get him to "enjoy" brushing
colgate spin brush
his favorite characters toothpaste
praising him for trying to brush his teeth
used a regular toothbrush
He HATES brushing his teeth... I am getting really concerned because I can see his teeth starting to develop a brown film on his front teeth 
and all the dentists in our area aren't accepting new patients until September!
I have tried to brush his teeth for him, but he hates that as well, and I try to make it a good experience but when I try and brush his teeth he seems frightened...
Any suggestions on how I can get his teeth clean so he doesn't develop any more of that yucky brown film??

BTW he can't talk, so he its really hard to communicate with him...
get a tooth tunes brush! works every time 
give him an inscentive; no candy/ desserts if he doesnt brush.
reward; praise every time he does brush.
